Job Title: Patient Navigator
Hours of Work:
16-22 hours per week - working schedule to be confirmed depending on agreed hours, working hours will be between 08:00-18:30.
You must have some flexibility to cover additional hours subject to business needs.
Accountable to: Practice Business Lead
Qualifications/Experience required:
Good level of general education including qualifications in English and Maths (equivalent to GCSE Grade C or above)
Experience of working with the public is essential. Experience within a reception or administrative role is highly desirable.
It would be advantageous for a candidate to be familiar with GP clinical systems, medical terminology and the NHS working environment.
Person Specification:
We are looking for a forward-thinking candidate with excellent communication skills, both written and verbal, with a passion for giving excellent customer service.
Our ideal candidate would have the ability to stay calm under pressure in a busy practice environment.
A polite, empathetic, and professional manner is essential.
Teamwork is important to us, the candidate must be able to work well with others and respond accordingly to unexpected circumstances.
IT skills are essential to the role and it would be advantageous to have a knowledge of MS Office. The NHS is a constantly changing environment and our ideal candidate would be adaptable and quick to accept system changes.
The ability to prioritise workload and manage multiple workstreams with a high level of accuracy is essential.
Discretion and confidentiality are essential qualities for this post.

Purpose of Job:
The role of Patient Navigator covers a range of duties, including but not limited to: clinical administration, reception, call handling, data manipulation and analysis, workflow management and other tasks essential to the day-to-day practice needs.
Key Aspects of Role
- Maintain medical records accurately
- Call handling, patient queries and appointment booking
- Data entry, manipulation and analysis
- Co-ordinate clinical tasks and medical referrals
- Manage appointments system and highlight concerns to management
- Deal with general enquiries from practices and other agencies
- Take concise messages with relevant details using appropriate channel
- Maintain patient safety and patient confidentiality
- Manage correspondence (postal and email)
- Scan and code documents in a timely and accurate manner
- GDPR compliance in line with practice guidelines
- Prepare appropriate documentation for audits
- Collate data for service reports
- Complete mandatory training and attend in house training as required
- Attend general staff meetings
- Maintain tidy workspace, complying with clear-desk policy
- Report all incidents or near miss incidents to line manager
- Any other duties deemed appropriate by line manager
Induction and Training
We offer comprehensive inductions and on-the-job training. The post-holder will participate in any training programme implemented as part of this employment. Training may include an annual individual performance review.
Confidentiality
Patient navigators will be dealing with confidential and sensitive information daily, therefore must maintain high levels of discretion.
Health & Safety
All employees must complete training in health and safety and are expected to highlight potential risks as appropriate, including infection control, manual handling and other key risks.
